Decoding the Czech Vaccination Schedule: A Guide for Expat Parents

Raising a child in a foreign country comes with a unique set of challenges. Just when you think you have mastered the local playgrounds and found your favorite family-friendly cafes, you are hit with a new bureaucratic hurdle: the Czech pediatric vaccination schedule (očkovací kalendář).
If you are an expat parent, comparing your home country's vaccine schedule to the Czech system can feel like deciphering a secret code. You might be wondering: Did my child miss a shot? Are they over-vaccinated? Will they be allowed to start kindergarten?
The Czech Republic has strict health laws, but the system is actually very straightforward once you understand the baseline.
Here is your clear, English-language guide to mandatory pediatric vaccines in Czechia.
1. The Golden Rule: "Mandatory" Means Mandatory
In many Western countries, vaccines are highly recommended but ultimately left to the parents' discretion. In the Czech Republic, a specific set of vaccines is legally mandatory (povinné očkování).
The Kindergarten Catch: This is where most expats get caught off guard. By law, Czech state preschools (mateřská škola or školka) and official children's groups (dětská skupina) cannot accept a child who has not completed the mandatory vaccination schedule (unless they have a certified medical exemption). If you plan to enroll your child in the local school system, having your paperwork perfectly aligned is not optional.
2. The Core Mandatory Vaccines
The Czech schedule focuses heavily on two main multi-dose vaccines during the first two years of a child's life:
The Hexavalent Vaccine (Hexavakcína): This 6-in-1 vaccine protects against Diphtheria, Tetanus, Whooping Cough (Pertussis), Haemophilus influenzae type B (Hib), Polio, and Hepatitis B.
Timeline: It is administered in a "2+1" scheme. The first dose is typically given from the 9th week of life, the second dose two months later, and the final booster between 11 and 13 months of age.
The MMR Vaccine (Spalničky, zarděnky, příušnice): This protects against Measles, Mumps, and Rubella.
Timeline: The first dose is given between the 13th and 18th month. The second dose is administered much later, between the 5th and 6th year of age.
Older Child Boosters: Additional mandatory boosters for Diphtheria, Tetanus, Whooping Cough, and Polio are required between ages 5–6 and again at ages 10–11.
3. Highly Recommended (But Optional) Vaccines
Alongside the mandatory list, Czech pediatricians strongly recommend several voluntary vaccines (nepovinné očkování). Because the Czech Republic has different endemic risks than the US or UK, these are highly worth considering:
Tick-Borne Encephalitis (Klíšťová encefalitida): Crucial if you plan to spend time in Czech parks or forests.
Pneumococcal & Meningococcal vaccines: Highly advised for infants to protect against severe bacterial meningitis and pneumonia.
Rotavirus: Administered orally to infants to prevent severe gastrointestinal illness.
4. The Expat Challenge: Merging Your Records
What happens if you move to Prague when your child is two years old, and your home country uses a 5-in-1 vaccine instead of a 6-in-1?
Do not panic. You do not need to restart the entire schedule. However, you do need a local pediatrician to officially review your foreign vaccination records, translate the equivalents, and create a custom "catch-up" plan to fulfill Czech legal requirements.
