2. Bring your insurance card
   Please remember to bring your valid insurance card to your first visit..

3. Payment guarantee
   If you are a client of an assistance company, please make sure a payment guarantee is provided for each visit..

4. Contact your assistance company
   To ensure a smooth process, ask them to send the payment guarantee to us before your appointment.
